RN-Home Health-Newcastle-WY

Provides skilled nursing care to patients in their homes, performing assessments, treatments and care coordination across an assigned caseload. Develops and updates the plan of care in collaboration with the physician and interdisciplinary team. Educates patients and caregivers to support safe self-management and independence in the home setting.

• Graduate of an accredited school of nursing:
  Required
• Home health, community or acute care nursing experience:
  Preferred

• Registered Nurse licensure in the state of practice:
  Required
• Basic Life Support (BLS) certification and a valid driver's license:
  Required

• Conduct comprehensive admission and ongoing assessments in the patient's home.
• Develop, implement and revise the plan of care in collaboration with the physician.
• Perform skilled nursing interventions, including wound care, medication management and IV therapy.
• Educate patients and caregivers on disease management, medications and safety in the home.
• Coordinate care with therapy, social work, home health aides and community resources.
• Complete timely, accurate documentation and participate in on-call coverage as scheduled.
