At Central Coast Community Women’s Health Centre (CCCWHC), our vision is safe and healthy futures for all women on the Coast. For almost 50 years, we have delivered holistic, trauma-informed health and wellbeing services for women, by women.

We are now seeking an Integrated Care Coordinator to join our Women’s Health Clinic in a newly established role that embeds social work and psychosocial support directly into primary health care. This is a rare opportunity to help shape how women experience care from the moment they walk through the door.

This position is offered on a two-year fixed-term contract, working four days per week, and is based at our Wyoming Women’s Health Centre.

About the role

The Integrated Care Coordinator is a clinic-based, social work–led role, working side-by-side with GPs, nurses and allied health staff as part of a multidisciplinary team.

You will provide in-practice psychosocial support, assessment and brief counselling for women accessing the clinic, particularly those experiencing domestic and family violence, chronic disease, reproductive health concerns, mental health challenges, and parenting pressures.

This role strengthens the connection between medical care and social support, ensuring women receive coordinated, trauma-informed care that responds to both their health and social needs. Your work will support safer, more seamless and more empowering care journeys.

What you’ll be doing

In this role, you will:

  • Provide warm, respectful and culturally safe engagement with women accessing the clinic
  • Conduct holistic, trauma-informed psychosocial assessments
  • Provide brief counselling and emotional support, including crisis response where required
  • Support women managing chronic disease and mental health concerns where social factors are impacting health outcomes
  • Screen for domestic and family violence and support safety and wellbeing planning
  • Assist women to navigate complex health and social systems, including Centre link, housing, legal and community services
  • Work collaboratively with GPs and allied health staff to identify and respond to psychosocial needs
  • Advocate for women within health, welfare and government systems
  • Independently manage a caseload and triage referrals with minimal direct supervision
  • Support continuity of care through coordinated referrals and proactive follow-up
  • Contribute to data collection, monitoring, evaluation and continuous service improvement
  • Participate in multidisciplinary meetings, reflective practice, supervision and professional development

What we’re looking for

You are an experienced practitioner who is confident working in complex, fast-paced health settings and who values integrated, relational care.

Education

Relevant tertiary qualification in Social Work, Nursing, Psychology, or another Allied Health discipline

Experience & skills

  • Minimum 2 years’ experience in counselling and/or casework
  • Demonstrated experience supporting women with complex psychosocial needs, including chronic disease, mental health and domestic and family violence
  • Strong knowledge of health and social support systems and confidence navigating these on behalf of clients
  • Ability to work autonomously, manage a caseload and triage referrals
  • Demonstrated experience working effectively within multidisciplinary teams, particularly alongside medical and clinical staff
