Position: MDS Coordinator (Sr. Nurse) 2026-00349

GENERAL DESCRIPTION

Join the team at our brand new skilled nursing facility and intermediate care facility located at the Wyoming Life Resource Center in Lander. We are looking for a Minimum Data Set (MDS) Coordinator who will be responsible for the MDS assessments of the residents at this facility. This position will work with the Interdisciplinary Team (IDT) to implement care plans and monitor the plans for effectiveness while assuring compliance with ethical standards, Federal and State regulations, and quality initiatives.

ESSENTIAL FUNCTIONS
  • MDS Duties – Establish the assessment reference date (ARD), reason for the assessment, accuracy, timely and accurate completion/submission of the assessments, including transmission of MDS’s to CMS.
  • Obtain the final validation reports for the MDS submission and ensure correction of all errors.
  • Ensure accuracy for all residents receiving Medicaid, as well as Medicare Part A and B benefits.
  • Participate in the screening and admissions of new residents.
  • Develop care plans and evaluate the efficacy of those care plans.
  • Ensure that baseline and comprehensive care plans are developed within timelines established by CMS regulations.
  • Update care plans as needed, ensuring completion of needed services.
  • Conduct care plan conferences with staff/residents/resident representatives in a timely manner, ensuring reimbursement for services.
  • Assist with scheduling resident assessment/care plan meetings.
  • Retrieve and review the Quality Indicator and Quality Measure Reports monthly, take necessary action, and report the information to the Quality Assurance Performance Improvement Committee.
  • Develop, implement, and maintain an ongoing quality assurance and performance improvement (QAPI) program for the resident assessment/care plans.
  • Review and stay informed on regulation changes relating to the RAI and reimbursement and implement changes.
  • Maintain and periodically update written policies and procedures that govern the use of the MDS and care plan.
  • Educate, train, and orient appropriate staff on completion of the MDS and use of the Resident Assessment Instrument Manual.
  • Other duties as assigned.
PREFERENCES

Preference will be given to applicants who are certified (RAC‑CT) in the MDS/RAI Process, and/or have worked in a skilled nursing facility and/or intermediate care facility for a minimum of one (1) year.

KNOWLEDGE
  • Knowledge of Wyoming Nurse Practice Act and Standards of Nursing Practice.
  • Detail oriented, directing others, and managing schedules for care planning in compliance with Medicare and Medicaid regulations.
  • Observe staff members providing care to ensure accurate reimbursement and compliance.
  • Oversee the assessment process and facilitate the completion of timely resident assessments.
  • Work well as part of a collaborative team.
  • Communicate effectively with staff members about residents.
  • Ensure staff are coding correctly.
  • Work with rehab teams to properly manage each resident's case.
  • Transmit accurate assessments in a timely manner.
  • Proficiency with Electronic Health Records (EHR), as well as other computer programs.
  • Coordinate care planning in accordance with regulatory requirements.
  • Initiate Medicare coverage and help determine documents needed for Medicaid reimbursement.
  • Knowledge of MDS requirements.
EDUCATION & EXPERIENCE

Bachelor's Degree (typically in Nursing)

1‑2 years of progressive work experience (typically in Nursing) with acquired knowledge at the level of a Nurse.
