• Evaluate and treat residents to improve mobility, strength, balance, and functional independence
• Develop and implement individualized treatment plans
• Document evaluations, treatments, and progress using Net Health and Point Click Care
• Collaborate with an interdisciplinary team to support resident-centered goals
• Bring professionalism, motivation, and a positive presence to every treatment session

• Active Pennsylvania Physical Therapist (PT) license
• SNF/LTC experience preferred
• Familiarity with Net Health and Point Click Care a plus
• Strong communication skills and a patient-focused approach
