• Assess referred patients requiring rehabilitation services; provide direct patient care, evaluate medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation.
• Establish and administer treatment programs with specific goals according to patient's capacity and tolerance under physician direction.
• Set realistic and achievable goals for patients, document and record condition, and educate patients and families in appropriate physical therapy methods.
• Develop plan of care for each patient.
• Provide skilled physical therapy services/interventions as per physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet therapy goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

• Graduated from an accredited school.
• Current state license as a Registered Physical Therapist/PT or proactively in the licensing process.
• Current CPR certification.
