A compassionate hospice care provider is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) in Wyomissing, Berks County. The ideal candidate will provide exceptional end-of-life care, work collaboratively with the Clinical Director, and support patients and families with compassion and professionalism. This role requires an active RN license, a minimum of one year's experience in medical surgical or acute care, and EMR proficiency.

Competitive compensation between $85,000 and $93,000 annually, with comprehensive benefits including health insurance and tuition reimbursement.
