Responsibilities
The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, following physician prescriptions and complying with relevant laws, regulations, and agency policies.
You will coordinate the overall care plan with other healthcare professionals, ensuring continuity of care by sharing information with physicians, agency staff, patients, families, and community resources—all within a Point of Care setting.
Responsibilities include conducting initial and comprehensive nursing evaluations, determining primary care focus, developing care plans within state guidelines, and submitting accurate documentation in collaboration with physicians.
You will perform comprehensive assessments at designated times and develop appropriate care plans, ensuring patients meet home health eligibility and medical necessity criteria as defined by payers.
Education and Experience
License Requirements:
- Current RN licensure in the state of practice
- Current CPR certification
- Valid Driver's License, vehicle insurance, and access to reliable transportation
