Holistic are Hospice is seeking a Per Diem Hospice RN Case Manager to join our compassionate nursing team delivering end-of-life care across the community.
As an RN Case Manager, you will coordinate services, support on‑call triage, and guide patients and families with clinical excellence and empathy.
Requirements: active RN license, at least 1 year in medical-surgical/acute care (hospice a plus), EMR proficiency, CPR if needed, and a valid driver's license with reliable transportation.
