We are hiring for:

Blended Case Manager / Salisbury Behavioral Health

Blended Case Management (BCM) assists children and adults with behavioral, medical, and developmental needs by building a strong network of community resources and services. The BCM program plays a major role in linking clients with needed services and providing 24/7 on‑call support for crisis intervention.

We are accepting applications for a Full Time Blended Case Manager who will serve individuals in Lehigh and Northampton Counties with behavioral, psychological, or developmental challenges.

Pay rate: $20.00/hr

Bonus: Employee sign‑on $1,000.00 paid in three increments: $250.00 after the 90‑day probation, $250.00 after six months, and $500.00 after twelve months.

Duties include:

  • Assisting consumers in gaining access to public and private resources/services such as medical, vocational, educational, employment, or other community resources as specified in the consumer’s service plan.
  • Communicating with consumers on a weekly basis to determine progress and revise the service plan as necessary.
  • Assisting consumers with resolution of problems and/or concerns.
  • Developing and enhancing a consumer’s social support network with families in the community, neighborhood, school, and workplace settings.
  • Observing and responding to significant changes in consumer mental and physical health and communicating changes to the consumer’s treatment team.
  • Collaborating with county Office of Mental Health personnel and other service providers to assess consumer needs and provide the most appropriate services.
  • Providing 24/7 on‑call crisis services on a rotational basis.
  • Maintaining case records and completing appropriate documentation, letters, assessments, and summaries as required by established policies and procedures.

75% of the work hours are out of the office in the community providing support.

This position offers participation in a bonus system based on performance.

Primary

Hours:

  • Full Time - 40 hours weekly – Hours vary – Flexible schedule

Requirements:

  • Bachelor’s degree in a human services related field (sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education)
  • Assumes an associate’s degree with 12 college credits relevant to the position (human services, psychology, sociology) and two years of experience in the mental health/human service field
  • Excellent communication, writing, and time‑management skills
  • Ability to establish and maintain effective working relationships with consumers, families, and staff from diverse cultures and social backgrounds
  • Valid Pennsylvania Driver’s License with a clean driving record
  • Ability to pass required background clearances and screenings
