Center Well Home Health is seeking a dedicated Home Health Occupational Therapist to help patients regain independence in their homes. You will assess, plan and administer rehabilitation programs, coordinating with physicians and the care team to optimize mobility and function.
This role includes a $10,000 sign-on or relocation assistance, tuition reimbursement, and a company vehicle with fuel benefits. You’ll contribute to care plans, discharge planning and community reintegration efforts.
