THIS IS FOR A MANAGER POSITION TO SUPERVISE THE RN/CM UNIT. SUPERVISORY/MANAGER EXPERIENCE REQUIRED.
Unit: Case Manager
Experience
Required:
5 years experience with at least 3-5 years of leadership experience. Seeking someone seasoned RN Case Management background, ideally with inpatient clinical experience. Case management and discharge planning, strong assessment skills, ability to discharge homeless patients and discharge patients to the next level of care to SNF, IP, LTAC, Shelter, Assisted Living Facility, Medicare/Medicaid patients.
Certs
Required:
BLS, BSN is required, Active WA/Compact license.
Travel Exp: No first time travelers
Patient Types/Common Diagnoses: Travelers will care for patients in NICU, pediatric, ER, Med Surg, Tele, Oncology, and ICU. No L&D.
Floating: throughout hospital.
Case Load: As low as 15 patients and as high as 25-30, depending on staffing.
