About the RoleOptum, a clinician‑led care organization, is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join our Home‑based Medical Care team. You will deliver high‑end medical service and compassionate care to complex, chronic patients in their homes, helping them remain independent and improving their quality of life.
Primary Responsibilities- Manage chronic illnesses in a complex elderly population, focusing on wellness and preventing unnecessary hospitalizations and ER visits.
- Provide patient education about disease processes, improving overall quality of care and patient satisfaction.
- Document timely care with a focus on closing HEDIS quality gap measures and recapturing HCCs.
- Engage new patients with a comprehensive geriatric assessment including functional, social, safety, cognitive and mental health screening.
- Triage, diagnose, and treat patients on an urgent basis in home using equipment and medications.
- Order and interpret appropriate tests (X‑rays, labs, EKGs) and prescribe treatment within the scope of practice.
- Initiate treatment in the home at the point of care.
- Collaborate with patients’ PCP and specialists as needed.
- Graduate from an accredited NP or PA program and hold certification from AANP, ANCC, or NCCPA.
- Active and unrestricted state Nurse Practitioner license or PA‑C in the state of employment.
- Valid DEA license or ability to obtain one prior to employment.
- Driver’s license and reliable transportation; if driving, comply with Optum Motor Vehicle Safety policy.
- 2+ years of clinical NP experience in IM, Geriatrics, Primary Care, ED, or Urgent Care.
- 1+ year of clinical experience as an NP or PA.
- Knowledge of UM standards, NCQA requirements, CMS guidelines, and Medicaid/Medicare contracts and benefit systems.
Compensation for this specialty ranges from $109,500 to $164,000, including base pay and bonus. Total cash compensation is based on local labor markets, education, work experience, and performance.
