About This Role
The Case Manager Liaison Nurse will provide and coordinate nursing care for pediatric patients in collaboration with the healthcare team. This role is ideal for experienced nurses looking to make a significant impact in outpatient complex care coordination.
Highlights- Pay: $49 - $82/hour
- Location: Renton, WA
- Schedule: Full Time, Day shift
- Ensure patients referred to case management meet established criteria.
- Assess patients' physical, mental, financial, and psychosocial status.
- Develop patient-centered case management plans with specific goals and outcomes.
- Evaluate the ability and availability of designated caregivers for patient support.
- Coordinate and implement interventions using evidence-based guidelines.
- Collaborate with healthcare professionals regarding the plan of care.
- Monitor and evaluate patient responses to therapeutic interventions.
- Facilitate referrals for home health care, long-term care, and hospice services.
- Participate in care conferences for patients with complex care needs.
- Collect data to evaluate the effects of care coordination on quality outcomes.
- Associate's Degree (ADN) required
- Pediatrics experience highly preferred
- Current RN License in Washington
