Position SummaryThe Family Care Coordinator is responsible for intensive case management, including classroom support and home visits, of children and families enrolled in the program. This role participates in recruitment and enrollment, screenings, care planning, classroom support, parent coaching, and connecting children and families with resources and services.
ResponsibilitiesFacilitate enrollment into the program with caregivers and child.
- Observe the parent-child interaction during intake and in the classroom.
- Complete enrollment paperwork including designated screening forms.
- Collaborate with transportation and classroom staff.
Provide ongoing case management and manage a caseload of children and families.
- Provide home visits with parents of enrolled children at least monthly to address family needs, positive parenting skills, and behavior planning consistent with the classroom and Family Care Plan..
- Coach caregivers on responsive parenting, child development, and social-emotional learning.
- Maintain consistent communication with families to monitor progress and provide ongoing support.
- Monitor family’s need for transportation and communicate with transportation liaison and document in child record.
- Support implementation of individualized behavior support strategies within classroom and home settings.
- Provide classroom observations and support; follow up with classroom staff regarding interventions, children’s progress or concerns, and family needs.
- Make referrals to community services whenever necessary.
- Maintain a caseload of 15-20 families.
Arrange multidisciplinary (MDT) meetings with parents, sources of referrals and allied service providers to establish a care plan for the child.
- Attend MDT meetings every 90 days to review the care plan including progress and new or ongoing needs..
- Ensure informal supports, schools, and allied services providers are included in the MDT.
Complete all documentation according to agency policy and required by the contract.
- Document all home visits, community meetings, MDT meetings and other contracts on calendar daily
- Complete documentation within 48 hours of scheduled visits
- Keep ELMS/Documentation tracker updated, as required
- Attend and plan for transition meetings in conjunction with Mental Health Consultant upon families approach and exit from ECLIPSE
- Document all phone calls, case notes, contacts with allied service providers and other treatment information in the electronic record according to the ECLIPSE policies and procedures.
- Maintain communication with assigned social workers and referral sources regarding progress and concerns.
- Follow up with classroom staff to make sure objective assessments are completed and scored on time.
- Meet productivity standards of 50% of time in direct face-to-face services including home visits, multidisciplinary team meetings, case management, classroom observations, etc.
- Maintain confidentiality according to HIPAA and FERPA in all aspects of the program
Actively participate in team operations and supervision
- Communicate directly with supervisor for any questions or concerns regarding program design/implementation, job requirements, behavior interventions, training or general program needs.
- Attend and participate in staff meetings, trainings, and individual and group supervision
- Staff cases that require team coordination or therapeutic input with Mental Health Consultant and/or Program Manager, as needed
- Organize and participate in family events, support groups, and parent meetings
