Nursing Assistant

Join Avera Health as a Nursing Assistant in our Long Term Care facility. In this role, you will provide compassionate, hands-on care to residents, assisting with daily living activities such as bathing, dressing, feeding, mobility, and toileting. You will obtain vital signs, report changes in condition to the nursing team, and help maintain a safe, clean, and dignified environment. Working closely with RNs, LPNs, residents, and families, you'll support individualized care plans while promoting comfort, respect, and independence.

Responsibilities

• Provide direct resident care and assistance with activities of daily living (bathing, dressing, feeding, toileting, grooming)
• Measure and record vital signs and report changes in residents' conditions to nursing staff
• Support residents with safe mobility, transfers, and use of assistive devices
• Maintain a clean, safe, and comfortable environment for residents
• Follow individualized care plans under direction of RNs and LPNs
• Document care provided according to facility policies and regulatory standards
• Assist with meal setup, feeding, and monitoring intake and output
• Communicate effectively with residents, families, and the care team
• Uphold infection control, safety, and confidentiality standards
• Demonstrate compassion, respect, and dignity in all resident interactions
