Home Health Registered Nurse

Become part of our caring community and help us put health first. As a Center Well Home Health nurse reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes.

Up to $10,000 sign‑on bonus.

• Provide skilled nursing care to patients, working onsite in their homes, including administering medications, performing wound care, and conducting patient assessments.
• Develop and implement individualized care plans with physicians and other healthcare professionals.
• Monitor patients’ conditions and report changes.
• Educate patients and families on disease management and treatment options.
• Maintain accurate records and coordinate with other professionals.
• Report patient care progress to physicians and the Clinical Manager.
• Oversee the work of Home Health Aides and ensure proper completion of documentation.

• Diploma, Associate, or Bachelor’s Degree in Nursing.
• Experience in medical‑surgical, ICU, ER, or acute care settings.
• Current, unrestricted Registered Nurse license.
• Current CPR certification.
• Experience collaborating with a multidisciplinary healthcare team.
• Valid driver’s license, auto insurance, and reliable transportation.

• One year of nursing experience.
• Experience in home health.

• $54.00 – $75.00 per visit/unit.
• $84,900 – $116,800 per year base pay.

TB Statement:
This role is patient facing and is part of Humana’s TB screening program. TB screening is required.

Driving Statement:
This role is part of Humana’s driver safety program and requires a valid state driver’s license, personal vehicle liability insurance, and adherence to state minimum limits.

40 hours per week.
