Job Summary:

Provide physical therapy services to patients in various clinical settings, focusing on restoring movement and function. Collaborate with healthcare teams to develop and implement individualized treatment plans ensuring optimal patient outcomes.

Responsibilities:

• Evaluate patients' physical conditions and establish treatment goals.
• Develop and implement customized therapy plans based on patient needs.
• Monitor patient progress and adjust therapy strategies accordingly.
• Document treatment procedures and patient responses accurately.
• Educate patients and families on therapeutic exercises and injury prevention.
• Comply with all documentation, licensing, and credentialing requirements.

Qualifications:

• Must hold an active state license in physical therapy.
• Prior experience in a clinical or outpatient setting preferred.
• Demonstrate strong communication and interpersonal skills.
• Ability to work effectively within a multidisciplinary team.
• Proficiency in documenting patient care and progress notes.
