Full Time 9am-5pm

Position Summary

The admissions Coordinator is responsible for managing the admissions process for prospective residents and patients, serving as a liaison between referral sources, families, and the nursing home team. This role ensures a smooth transition into the facility while maintaining high occupancy and excellent customer service.

Key Responsibilities
  • Coordinate and manage resident admissions from hospitals, rehabilitation centers, and community referrals.
  • Communicate with prospective residents, families, and referral sources regarding services and admission requirements.
  • Verify insurance benefits and obtain necessary documentation.
  • Conduct facility tours and answer questions about care and services.
  • Collaborate with nursing, social services, and administration to ensure a seamless admission process.
  • Maintain accurate admission records and census data.
  • Develop and maintain positive relationships with hospitals, physicians, case managers, and community partners.
  • Assist with marketing and outreach efforts to increase referrals and occupancy.
Qualifications
  • High school diploma required; associate or bachelor's degree preferred.
  • Previous healthcare, admissions, customer service, or long-term care experience preferred.
  • Knowledge of Medicare, Medicaid, and insurance processes preferred.
  • Strong communication, organizational, and interpersonal skills.
  • Proficient in Microsoft Office and electronic medical records systems.
