What You’ll DoUnder the direction of the Coordinator of Rehab Services, you will deliver rehabilitative and range‑of‑motion services to individuals served, while playing a key role in equipment management, evaluations, and staff training.
Your Impact Will Include- Providing range of motion and rehabilitative services as referred by the Interdisciplinary Treatment Team (ITT), using skilled manual techniques and safe lifting (up to approximately 60 lbs).
- Assisting with equipment weaning, maintaining schedules, and monitoring progress to support functional outcomes.
- Supporting the Physical Therapist by coordinating and securing clinic visits related to equipment concerns, and accompanying individuals to appointments as needed.
- Ensuring all required documentation and paperwork are prepared and transported for clinic visits.
- Assisting with annual and quarterly evaluations, including:
- Transporting individuals in wheelchairs (combined weight up to approximately 140 lbs)
- Observing individuals in adaptive equipment
- Taking goniometer readings of all extremities
- Bending, kneeling, stooping, and visually assessing positioning and alignment
- Providing weekly observations of individuals on assigned teams and submitting written recommendations to leadership.
- Delivering hands‑on training and in‑service follow‑up to direct care and clinical staff on positioning, equipment application, and range of motion techniques.
- Attending annual evaluations and presenting oral reports in the absence of the Physical Therapist.
- Applying, monitoring, adjusting, repairing, or altering adaptive equipment to ensure proper fit, safety, and condition.
- Completing designated sections of Medicaid Justification Forms accurately and on time.
- Serving as a clinical resource on rehabilitative and mobility‑related issues.
- Current New York State licensure as a Physical Therapist Assistant
- Strong clinical observation, documentation, and communication skills
- Comfort working in a hands‑on, physically active role
- Ability to collaborate with an interdisciplinary team

- Ability to meet the physical demands of the role, including lifting, bending, standing, kneeling, and manual dexterity
