Monday-Friday, generally 9:00 a.m.

-5:00 p.m.; in-person coverage one weekend per month, with schedule adjustment during that week

St. Clare Friary is a private assisted-living community serving Franciscan friars/priests in their later years. Our small residence
-typically 7 to 12 residents- offers an opportunity to provide attentive, relationship-centered support.

We are seeking an experienced, compassionate healthcare professional to serve as our Assistant Director/Care Manager. This is an ideal role for a nurse or medical social worker/care manager with substantial experience in senior care who can bring sound judgment, leadership, and practical skills to a mission-driven environment.

Working closely with the Healthcare Director, the successful candidate will help coordinate resident care, guide and support staff, strengthen safety and quality practices, and serve as a trusted day-to-day presence for the friars.

St. Clare Friary is a Catholic, Franciscan community. The ideal candidate will be one who understands and respects Catholic faith, Franciscan values, and prayerful community life. Candidates should be comfortable working in a religious residence.

Resident care coordination and advocacy
  • Monitor residents’ changing needs and participate in care-planning discussions with the Healthcare Director and outside providers.
  • Coordinate communication with physicians, hospitals, pharmacies, home-care agencies, and other healthcare partners.
  • Participate in admissions, transitions of care, hospital discharges, and changes in level of service.
  • Help maintain complete, accurate, confidential resident records, including service plans, emergency contacts, advance directives, and other required documentation.
  • Serve as a calm, accessible resource to residents and staff.
Operations, quality, and safety
  • Assist the Healthcare Director with the daily operations of the residence and make appropriate operational decisions in the Director’s absence.
  • Help develop, implement, review, and monitor policies and procedures related to resident care coordination, staffing, safety, emergency preparedness, documentation, and infection prevention.
  • Track, review, and address incidents, falls, medication-related concerns, hospital transfers, and other quality indicators.
  • Assist with basic budget oversight, purchasing, and expense monitoring.
Staff leadership and support
  • Assist with onboarding, scheduling, orientation, supervision, and evaluation of non-licensed care and support staff, in collaboration with the Healthcare Director and Human Resources.
  • Reinforce staff expectations related to resident dignity, safety, infection prevention, documentation, and respectful communication.
  • Provide coaching, feedback, and problem-solving support to staff members.
  • Help foster a culture of teamwork, reliability, and professionalism.
Daily life in the friary
  • Be a visible and supportive part of daily life in the community, including frequent participation in lunch with the friars.
  • Assist residents with ADLs, IADLs, mobility, medication-related tasks, and transportation coordination when home health aides are unavailable and within the candidate’s qualifications and the organization’s policies.
  • Help arrange transportation to appointments and serve as a backup driver when needed.
