Nurse Practitioner, IDD Community
Our client, a dedicated non‑profit organization supporting individuals with developmental, intellectual, and behavioral challenges in Westchester County, is hiring a Nurse Practitioner, IDD Community.
Provide vital medical care and support to individuals with developmental disabilities in a collaborative, community‑focused environment. This role offers the opportunity to make a meaningful difference in the lives of those served while working alongside a compassionate team of professionals.
Responsibilities- Conducting annual and as‑needed physical examinations, including vision and hearing screenings.
- Writing prescriptions, medical orders, and immunizations following established protocols.
- Communicating with guardians and families regarding medical emergencies and significant health changes.
- Evaluating and monitoring individualized habilitation programs in partnership with rehabilitation staff.
- Documenting all medical assessments, histories, and treatments accurately in electronic health records.
- Coordinating with healthcare providers at external facilities for continuity of care.
- Performing initial assessments for new admissions and providing medical updates to team leaders.
- Ordering and reviewing diagnostic tests and consulting with specialists as necessary.
- Developing diagnoses and treatment plans, including management of medication regimens such as seizure control.
- Schedule:
Full‑time, Monday through Friday, 8 am to 4 pm with flexibility.
- Graduate of an accredited nursing program.
- Valid NY State Nurse Practitioner license.
- One or more years of direct experience with individuals with developmental disabilities.
- Strong communication skills and ability to collaborate effectively within a multidisciplinary team.
- Knowledge of clinical protocols for individuals with intellectual and behavioral challenges.
- Ability to work independently, having completed at least 3,600 hours of supervision in a no collaborative agreement capacity.
- Proficiency with EMR systems and e‑prescribing platforms.
- Prior experience working within OPWDD or similar developmental disability programs.
