Location: City of Yonkers

The Wound Care Registered Nurse serves as a clinical expert, educator, and resource for the prevention and management of skin breakdown, wounds, ostomies, and continence issues across all hospital units. This role ensures evidence-based wound care practices that promote optimal healing and patient outcomes through direct patient care, staff education, interdisciplinary collaboration, and policy development.

Schedule:

days vary with alternating weekends.

Responsibilities
  • Assess, treat, and monitor acute and chronic wounds, developing individualized care plans.
  • Perform wound care procedures, including dressing changes, debridement, and ostomy management.
  • Conduct wound rounds throughout the hospital and collaborate with interdisciplinary teams to optimize patient care.
  • Educate patients, families, and staff on wound prevention, treatment, and infection control practices.
  • Document patient progress and adjust treatment plans as needed.
  • Develop and implement wound care policies, procedures, and educational resources.
  • Participate in committees, quality improvement initiatives, and root cause analyses related to patient safety and wound management.
  • Monitor and report wound-related quality indicators and sentinel events.
  • Manage wound care supplies and ensure availability of necessary resources.
  • Maintain current knowledge of wound care standards, research, and best practices.
Qualifications
  • Bachelor’s degree in nursing (BSN) required
  • Wound Ostomy Continence Nurse Certification (CWOCN) highly recommended, plus specialized certifications as needed.
  • Minimum of 2 years of wound care experience in a clinical setting.
  • Knowledge of health‑care data standards and regulations (e.g., HIPAA).
  • Strong knowledge of wound care practices and protocols.
  • Excellent communication and presentation skills.
  • Effective team collaborator with the ability to work independently.
  • Detail-oriented with strong organizational and documentation abilities.
