Travel Nurse - RN - Home Health

Role Overview

A Home Health Registered Nurse (RN) provides direct patient care in home settings, focusing on assessment, treatment, education, and care planning.

• Assess patient conditions and develop individualized care plans
• Administer medications, treatments, and wound care
• Educate patients and caregivers on disease management and self-care techniques
• Monitor patient progress, report changes, and adjust care plans accordingly
• Coordinate with healthcare professionals to ensure seamless care transitions
• Document interventions and patient responses accurately and comply with regulations
• Maintain infection control, safety, and infection prevention standards
• Support families and caregivers emotionally and through guidance
• Communicate with interdisciplinary teams and complete required documentation
• Perform additional duties within scope of practice as needed

• Minimum one year recent RN experience, including six months in home health
• Proficiency in OASIS documentation
• Valid RN license per state regulations
• BLS certification (AHA/ARC)
• Strong critical thinking, communication, and interpersonal skills
• Physical ability to perform patient care tasks, including lifting and moving equipment
• Ability to work effectively under pressure and adhere to safety standards
• Proficiency in written and verbal English, with good motor and sensory skills
