Description
This unique position’s primary function is to fulfill Catholic Charities commitment to serve individuals and families in an inclusive and holistic manner. An integral part of the Case Management team, this position will be based in Westchester. The position provides direct assistance for those in need through comprehensive case management services.
Responsibilities- Provide advanced case management to individuals and families, addressing key social determinants of health such as housing, food, income, and healthcare access. Provide effective engagement, assessment, service planning, referral and follow-up using a client-centered approach.
- Provide crisis intervention in emergencies around shelter, food, clothing, and other basic needs through the provision of financial assistance, goods, advocacy, benefits access and access to relevant resources.
- Consistently conduct thorough assessments and develop a service plan that addresses immediate needs and longer-term goals.
- Develop knowledge about and maintain collaborative relationships with community agencies, other Catholic Charities Departments, Divisions, Affiliates and Archdiocesan entities.
- Develop a capacity to provide or ensure access to a wide range of social service benefits and help develop and maintain a network of high-quality resources.
- Maintain accurate case notes and work across platforms to meet program deliverables and support reporting to funders and other stakeholders.
- Track authorization. Actively participates in weekly supervision.
- Attend and participate in monthly department meetings, community meetings, trainings, and other agency-related activities.
- Fulfills operational and other duties as assigned.
This is a full-time position. Days and hours of work are generally 9:00 AM to 5:00 PM, Monday – Friday. Additional hours may be required to meet program deadlines or client needs. There may be periods when work hours vary.
Salary and LocationSalary: $32.97 per hour, non-exempt based on a 35-hour work week, or $60,000 annually.
Location: 204 Hawthorne Avenue, Yonkers, NY, 10705.

- Remain in a stationary position at a workstation and use a computer approximately 60% of the time.
Education and/or experience required:
- Bachelor’s degree in the social services field with at least 3–5 years of experience in case management and social services.
- Bilingual English/Spanish required.
- Excellent interpersonal skills; ability to work well independently and as part of a team.
- Knowledge of social service delivery systems and local resources.
- Excellent organizational, time-management and communication skills.
- Knowledge of Unite Us platform is a plus.
