Westchester Disabled on the Move is seeking a part-time Transition Specialist in Yonkers, NY to assist consumers at risk of nursing facility placement. You will help procure services and benefits to remain in or return to the community.
The role reports to the Housing Manager and requires coordination with discharge planners, care managers, and service coordinators. A bachelor’s or associate degree with relevant experience is preferred, with strong communication and organizational skills.
