Case Manager

Westchester Jewish Community Services (WJCS) is an award-winning nonprofit, nonsectarian human services organization founded in 1943. Today, it has grown to be the largest provider of licensed outpatient community behavioral health services and one of the largest human service agencies in Westchester County. WJCS delivers innovative programs and services and compassionate care to more than 20,000 people of all ages and backgrounds throughout the county.

Join the team at the Mary J. Blige Center for Women, where you will make a meaningful impact as a Case Manager. In this role, you will be responsible for engaging and enrolling referred individuals into appropriate services, completing initial and ongoing assessments, and developing individualized service plans. The Case Manager will work collaboratively with clients to address their most urgent needs, including housing and food insecurity, affordable childcare, transportation linkage, legal resources, clothing, personal care, and assistance with Medicaid and other entitlements.

The Case Manager will ensure accurate Medicaid billing in compliance with established protocols and meet productivity standards by efficiently managing caseloads, documenting case activities, and maintaining timely follow-ups. The salary range for this full-time (35 hours per week) position is $50,000-$52,000.

Responsibilities:

As a valued member of our team, your responsibilities will include, but are not limited to:

  • Engage and enroll referred individuals into appropriate services through compassionate, culturally sensitive outreach and engagement strategies
  • Conduct comprehensive initial assessments and ongoing evaluations to identify clients' needs, strengths, and barriers
  • Develop and implement individualized service plans in collaboration with clients, prioritizing urgent needs such as housing, food security, transportation, legal resources, and access to benefits
  • Provide in-person support and case management services during regular hours and late evenings to accommodate clients' schedules and improve access
  • Perform field outreach activities by visiting community locations, shelters, and other sites to connect with clients who may have difficulty accessing in-office services
  • Assist clients with securing and maintaining housing, applying for Medicaid and other entitlements, and accessing community resources such as clothing, personal care items, and childcare
  • Support clients in overcoming barriers related to mental health and substance use, fostering empowerment and self-sufficiency
  • Ensure accurate and timely documentation of all case activities, assessments, service plans, and follow-ups in compliance with organizational policies and federal/state regulations
  • Manage caseload effectively to meet productivity standards, ensuring consistent follow-up and progress tracking
  • Ensure accurate billing for Medicaid and other reimbursements, adhering to established protocols and documentation requirements
  • Collaborate with internal teams, community agencies, and service providers to coordinate care and enhance service delivery
  • Maintain confidentiality and adhere to all organizational policies, federal and state laws, and quality assurance standards
  • Support clients during late evening hours as needed, providing flexible, accessible services to meet diverse client needs
Requirements:

We are seeking candidates who possess the following qualifications:

  • Bachelors degree in Social Work or a related field
  • Familiarity with community resources and case management practices for high-risk populations
  • Bilingual English/Spanish proficiency
  • Strong interpersonal, engagement, and communication skills
  • Ability to travel to different sites
  • Proficiency with computer skills
