Care Manager - Registered Nurse

• 12 months of role experience is required with some in the last 12 months.
• Must have at least 36 months of Case Management experience.
• BSN completion required.

• Candidates must have a Maine license or compact license (required for submission).
• Local & travel allowed.
• COVID vaccination required after submission. Religious and medical declinations accepted.
• Flu vaccination required for submission. Declinations not accepted.
• No current placement allowed at Program:
  Medefis Program - York Hospital.

Required Skills /

Experience:

Registered Nurse with 3-5 years clinical experience in medical/surgical nursing, home care, hospital case management and/or previous utilization review experience. Must have utilization review and discharge planning experience.

Required Credentials: Education:
Associate or Bachelor’s Degree.

Shift & Scheduling: Either 5x8 (8:30 to 4:30) or 4x10 that will start at 7:30 am - 5:00 pm. No Call. Weekend and Holiday Rotation is required. 7 days maximum time off. 1 reference from last 12 months (Manager/Supervisor reference type) - required for submission. Proof of identification required.
