Role Overview

The RN Care Manager coordinates concurrent utilization management for hospital patients, serving as the primary link among:

  • Patients
  • Healthcare team
  • Organization
  • Insurance payors

The role involves facilitating smooth patient transitions, resource coordination, and communication across teams.

Key Responsibilities
  • Assess patient needs and coordinate resources for transitions of care
  • Lead and educate interdisciplinary care teams during rounds, care conferences, and care planning
  • Communicate effectively in writing, verbally, and electronically
  • Work with community organizations for:
    • Durable medical equipment
    • Insurance authorizations
    • Transportation arrangements
  • Monitor patient length of stay and coordinate care accordingly
  • Apply knowledge of hospital reimbursement and quality measures
Qualifications

Education & Licensure
:

  • Associate or Bachelor’s degree in Nursing
  • Active RN license
  • Active BLS required

Experience
:

  • 3–5 years in medical/surgical nursing, home care, case management, or utilization review
  • Utilization review and discharge planning experience required

Preferred Certifications
:

  • Certified Case Manager (CCM) or Accredited Case Manager (ACM)
  • Certification from a nationally recognized case management or utilization review organization
