- Provides care across hospital, community sites, and physician practices
The RN Care Manager coordinates concurrent utilization management for hospital patients, serving as the primary link among:
- Patients
- Healthcare team
- Organization
- Insurance payors
The role involves facilitating smooth patient transitions, resource coordination, and communication across teams.
Key Responsibilities- Assess patient needs and coordinate resources for transitions of care
- Lead and educate interdisciplinary care teams during rounds, care conferences, and care planning
- Communicate effectively in writing, verbally, and electronically
- Work with community organizations for:
- Durable medical equipment
- Insurance authorizations
- Transportation arrangements
- Monitor patient length of stay and coordinate care accordingly
- Apply knowledge of hospital reimbursement and quality measures
Education & Licensure
:
- Associate or Bachelor’s degree in Nursing
- Active RN license
- Active BLS required
Experience
:
- 3–5 years in medical/surgical nursing, home care, case management, or utilization review
- Utilization review and discharge planning experience required
Preferred Certifications
:
- Certified Case Manager (CCM) or Accredited Case Manager (ACM)
- Certification from a nationally recognized case management or utilization review organization
