• Travel assignment in a hospital setting with care management responsibilities

• Conduct assessments of patient needs to identify resources required for transition to the next setting and secure those resources
• Coordinate concurrent utilization management activities and participate in discharge planning across the patient care team
• Lead interdisciplinary rounds, care conferences, and planning to align care with patient goals and payer requirements
• Educate and collaborate with the care team and community resources to arrange durable medical equipment, transportation, and insurance authorizations
• Maintain clear documentation in the electronic health record and communicate effectively through standard hospital systems

• Active Registered Nurse license in Maine
• Associate or Bachelor degree in nursing
• 3–5 years of clinical experience in medical/surgical nursing, home care, hospital case management, and/or utilization review
• Utilization review and discharge planning experience required
• BLS certification required; CCM or ACM certification preferred
• Ability to work in a hospital inpatient setting and meet travel assignment requirements

• Guaranteed hours per week
• Travel assignment with dedicated recruiter support from Healthforce Healthcare LLC
• Ongoing assignment support throughout the 13‑week engagement
