DESCRIPTION OF WORK
As an Aging Case Aide 2, you will be responsible for supporting the Social Services Department staff, including the Care Management Unit, the Intake and Assessment Unit, and the Waiver Unit. This role uses the statewide Department of Aging database known as Well‑Sky. Work duties involve receiving and reconciling service status sheets, contacting consumers by telephone to determine the status of services and satisfaction, fielding all incoming telephone calls from consumers who have questions about Aging Programs, and assisting any walk‑in applicants.
On occasion, you will conduct home visits in the community to provide certain applications to be completed by consumers in their own home.
- Review local obituaries and local newspapers to compare the obituaries to the consumer database.
- Record and create in the Well‑Sky database any consumer activity that takes place under the umbrella of Person-Centered Counseling.
- Pick‑up or deliver consumer necessities as requested by the Care Managers and Supervisors.
Work Schedule and Additional Information:
Part-time employment, Work hours are 9:00 AM to 2:00 PM, Monday – Friday, with a 30-minute lunch. Telework:

In some cases, the starting salary may be non‑negotiable.
- One year as an Aging Case Aide 1; or
- Two years of public contact work, including one year in a human services program; or
- Any equivalent combination of experience and training.
You must be able to perform essential job functions.
