The Case Manager Role

As a County Programs Case Manager within our County Programs team, you’ll play a vital role in supporting individuals through person-centered mental health screenings, intakes, and service coordination. This position is ideal for someone who is tech-savvy, resourceful, and adaptable—and thrives in fast-paced environments where collaboration and responsiveness matter.

Starting Rate: $19 hourly

Case Manager Core Responsibilities
  • Schedule and complete mental health intakes
  • Verify MA status and insurance eligibility
  • Complete service authorizations and Individual Service Plans
  • Maintain a caseload and complete required documentation
  • Advocate for the needs of persons served and coordinate referrals
  • Assess individual needs and gather prior treatment history
  • Support person-centered planning in accordance with county guidelines
What You Bring
  • Bachelor's degree in psychology, social work, counseling, social welfare, sociology, vocational rehabilitation, or a related field; OR
  • High school diploma with 12 college credits in relevant social sciences and two years of direct mental health experience
  • Ability to communicate effectively, work independently, and document clearly
  • Responsive, ethical, and kind with strong problem-solving skills
  • A valid Pennsylvania driver’s license and reliable transportation

Location & Schedule

This position is based in Lewistown, Pennsylvania. Work is primarily in-office with some community based expectations. Office hours are Monday through Friday, 8:30 AM to 5:00 PM.
