Job Summary
Join Geisinger’s Care Coordination team as an RN Hospital Case Manager and take the next step in your nursing career. This is an in‑person RN position based at Geisinger Medical Center (GMC) providing on‑site support to inpatient care teams. You will work Monday through Friday, 8:00AM to 4:30PM, with occasional coverage of weekends as part of a shared rotation (approximately every fourth weekend).
The position is exempt and salaried.
- Assess, plan, implement, coordinate, monitor and evaluate all options and services to optimize patient or member health status.
- Manage utilization and practice metrics to refine the delivery of care and maximize clinical, quality and fiscal outcomes.
- Integrate evidence‑based clinical guidelines, preventive guidelines and protocols in the development of patient‑centric treatment plans.
- Develop and monitor systems of care that promote early intervention in acute care situations.
- Assist with the design, implementation and evaluation of the advanced patient‑centered care model.
- Assess the healthcare, educational and psychosocial needs of patients or members.
- Design an individualized plan of care and foster a team approach by working collaboratively with the patient or member, family, primary care provider and other health‑care team members.
- Continuously evaluate laboratory results, diagnostic tests, utilization patterns and other metrics to monitor quality and efficiency.
- Apply benefits and utilization management appropriately, serving as a resource to the patient or member and the health‑care team.
- Maintain required documentation for all case management activities.
- Collect required data and adjust the treatment plan when necessary.
- Work in a clinical environment and comply with all organization policies and procedures.
- At least two (2) years of registered nurse (RN) experience required.
- Prior RN experience in a hospital setting preferred.
- Case management experience is preferred but not required.
- Licensed Registered Nurse (Pennsylvania) – of Pennsylvania.
- Graduate from a Specialty Training Program‑Nursing (Required).
- Minimum of 2 years of nursing experience (Required).
