PurposeEnnoble Care is hiring a full‑time Hospice RN Case Manager who resides within or close to Chester / Montgomery County, PA. The RN Case Manager works under the direction of the Director of Nursing, attending physician, and interdisciplinary team to perform skilled nursing assessments, planning, and care to maximize comfort and health for patients and their loved ones or caregivers.
What You’ll Do- Manage your assigned patient caseload, assessing, planning, implementing, and evaluating patient care throughout all phases of the nursing process.
- Document all notes in the EMR accurately and timely.
- Communicate and partner cross‑departmentally with physicians, hospice staff, and other agencies to coordinate optimal care.
- Educate and coach relevant parties on call levels of care for the patient, including physical, mental, emotional aspects.
- Partner with clinical leadership to address issues, provide professional support, and exchange feedback to enhance growth.
- Attend patient care conferences and facilitate discussion of caseload issues.
- Supervise and document the NAC every two weeks or as assigned.
- Assume the associate nurse role when responding to patient/loved ones/caregivers of other primary nurses during off‑hours or at the request of the Patient Care Coordinator.
- Provide support at the time of death and for bereavement of family members.
- Participate in hospice orientation, in‑service training, and community programs related to hospice care.
- Maintain familiarity with agency policies and state/federal regulations governing hospice.
- Accept other assignments as appropriate.
- Valid State license to practice as a Registered Nurse in Pennsylvania.
- 1–2+ years of professional nursing experience, preferably including hospice care.
- Ability to assess and respond to patient and caregiver needs in varied settings.
- Proficiency in nursing practice and ability to cope with caregiver emotional stress.
- Skilled in making meaningful observations, assessments, and comprehensive, accurate reports.
- Team‑player who supports interdisciplinary care for patients and loved ones.
- Valid driver’s license, good driving record, reliable vehicle, and ability to drive year‑round to locations.
- Capability to take direction, work independently, and function effectively within a multidisciplinary team in a professional manner.
