** What You'll Champion:*
* + Advanced Practice Provider (APP - NP/PA) position supporting Well Span Physiatry patients at Well Span Surgery and Rehabilitation Hospital with consultative coverage at Well Span York Hospital

+ Saturdays and Sundays, working 1 p.m.

- 11 p.m.

+ Seeking candidates with prior Physical Medicine & Rehabilitation (PM&R) / Physiatry experience

+ Hospitalist or inpatient care experience strongly preferred

+ Must demonstrate a solid understanding of inpatient workflows, interdisciplinary care coordination, and transition planning

+ Coordinate inpatient rehabilitation care under the supervision of attending physiatrists, including admission and discharge medication reconciliation, ordering diagnostics, consults, and facilitating therapeutic services

+ Perform rehabilitation consult evaluations for hospitalized patients and provide recommendations on level of care, IRF placement, and physiatry-specific management

+ Collaborate daily with attending physicians and interdisciplinary teams to review vitals, labs, care plans, provide handoff/sign-out, and maintain ongoing provider communication

+ Non-admitting role with structured onboarding; APPs work in direct daily communication with attendings, with access to hospitalist support and consult attending oversight
