Medley Health is seeking a compassionate and motivated Nurse Practitioner with PM&R (Physical Medicine & Rehabilitation) experience to join our growing post-acute care team. This is a part-time opportunity with schedule flexibility. You'll provide on-site physiatry and pain management services across skilled nursing and rehabilitation centers in Pennsylvania, helping patients regain function and improve quality of life.

• Conduct comprehensive physiatry consultations and follow-up care
• Collaborate with nursing, therapy, and clinical teams to optimize patient outcomes
• Manage rehabilitation, pain management, and functional restoration plans
• Provide guidance on patient care and progress to interdisciplinary teams

• NP board certification required (ANCC/AANP)
• Active Pennsylvania NP license required
• Experience in PM&R, physiatry, or pain management strongly preferred
• Strong communication and collaboration skills
• Passion for improving patient quality of life in post-acute and long-term care settings
