In this position, you will support the delivery of medical and psychiatric care for patients receiving treatment within a designated service area. You will promote effective assessment, planning, and coordination of care. As a Certified Registered Nurse Practitioner, you will perform the following duties:
- Patient Evaluation – Conduct medical assessments and diagnostic evaluations
- Clinical Documentation – Complete required patient records in alignment with DHS and hospital standards
- Treatment Leadership – Direct and participate in treatment team activities to guide care planning
- Medical Monitoring – Review laboratory findings, diagnostic reports, and clinical data to identify notable conditions
- Care Coordination – Communicate with families, staff, and community partners to support treatment goals
- Discharge Planning – Prepare summaries and coordinate with service units to ensure continuity of care
- Full‑time employment
- Work hours are 8:00 AM to 4:00 PM, Monday – Friday, with a 30‑minute lunch
- Telework – You will not have the option to telework in this position
- Salary – In some cases, the starting salary may be non‑negotiable
- Valid certification as a Certified Registered Nurse Practitioner (CRNP) issued by the Pennsylvania State Board of Nursing
- Valid CRNP certification in the Adult Care or Family Care specialty area as issued by the Pennsylvania State Board of Nursing
- Valid CRNP Prescriptive Authority Approval to prescribe and dispense drugs as issued by the Pennsylvania State Board of Nursing
- PA residency requirement is currently waived for this title
- Ability to perform essential job functions
- Conditional offer of employment will require a medical examination
- This position falls under the provisions of the Older Adult Protective Services Act
- Under the act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to PA State Police and FBI clearance
