About the Licensed Practical Nurse LPN Role:

As a Licensed Practical Nurse (LPN) Per Diem at Pleasant Acres Nursing & Rehabilitation Center, you will:

• Provide direct patient care to our residents, including vital sign assessments, medication administration, and wound care

• Collaborate with our interdisciplinary team to develop and implement individualized care plans

• Monitor and report changes in resident condition to the charge nurse and/or physician

• Participate in Quality Improvement initiatives to ensure the highest standards of care

• Demonstrate a strong commitment to patient safety and falls prevention

Licensed Practical Nurse LPN Responsibilities:

• Active and unrestricted LPN license in the state of Pennsylvania

• Prior experience preferred; new graduates welcome

• Passion for delivering exceptional patient care and customer service

• Ability to work a flexible per diem schedule, including days, evenings, and weekends

• Strong communication and collaboration skills

• Proficiency in electronic medical records and other software applications
