** What You'll Champion:*
* + Seeking a Primary Care Family Nurse Practitioner (FNP) to support a Community Health Center dedicated to improving access to care in York County.

+ Provide continuity of care for an existing and growing patient population.

+ Manage and support an established patient panel, including individuals receiving Medication‑Assisted Treatment (MAT).

+ Help establish care for a vulnerable and underserved patient population in a mission‑driven outpatient setting.

+ Deliver both in‑person and virtual primary care services, supporting flexible access for patients.

+ Collaborate within a supportive care team consisting of two CRNPs and one physician.

+ Practice in a team‑based, integrated primary care environment focused on patient‑centered outcomes.

Schedule:

+ Monday: 8:00 AM - 4:30 PM

+ Tuesday: 11:00 AM - 7:00 PM Includes virtual patient care hours from 5:00 PM - 7:00 PM

+ Wednesday: 8:00 AM - 5:00 PM

+ Thursday: 10:00 AM - 2:00 PM

+ Friday: 8:00 AM - 4:30 PM
