Registered Nurse LTC​/SNF

Job Summary:

Provide high-quality nursing care to residents in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF) setting. Deliver comprehensive patient assessments and coordinate multidisciplinary care to ensure resident well-being.

Responsibilities:

• Assess residents’ health status and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor residents’ conditions and respond to changes promptly.
• Collaborate with healthcare team members to coordinate care.
• Maintain accurate and detailed medical records in compliance with regulations.

Qualifications:

• Valid RN license in the state.
• Previous LTC and PCC experience preferred.
• Ability to assess and prioritize patient needs effectively.
• Strong communication and interpersonal skills.
• Familiarity with regulatory guidelines for LTC/SNF facilities.
• Reliable with the ability to work a 40-day contract with notice requirements.
