Bell Socialization Services Inc., a nonprofit leader in mental health, intellectual disability, and shelter services, is seeking a Case Management Assistant to support our Community & Social Services team. In this role, you'll assist case managers with client intakes, service coordination, scheduling, documentation, and follow-up to ensure individuals receive timely support and resources. You'll maintain accurate records, communicate with clients and providers, and help monitor service plans.

Responsibilities

  • Assist case managers with client intakes, assessments, and service plan follow-up
  • Coordinate and schedule client appointments, meetings, and referrals with community providers
  • Maintain accurate and timely client records and documentation in electronic and/or paper systems
  • Gather and organize required documentation for benefits applications, housing, and other services
  • Track referrals and services to ensure follow-through and completion, updating case managers regularly
  • Communicate with clients, families, and partner agencies via phone, email, and in-person contact while maintaining confidentiality
  • Support clients in understanding and following service plans by providing reminders and basic guidance
  • Prepare summaries, reports, and updates for case managers and supervisors as requested
  • Participate in home visits or community-based meetings as needed to support service coordination
  • Adhere to agency policies, ethical standards, and all applicable regulations related to client privacy and professional conduct
