We are currently seeking a Transition Coordinator to join our Transitional Services program. As a Transition Coordinator, you'll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. You'll be part of a mission-driven team that values your expertise, supports your growth, and empowers you to make a real difference in your community.
- Act as point-of-contact to assist the individual to live in his/her home and/or community.
- Assist individuals served with linkages to service providers and community resources as needed.
- Identify barriers to compliance with individual's healthcare regime, appointments and screenings.
- Work with individuals to create solutions to identified barriers and empower individuals to manage physician, and/or other key medical professionals, contacts, appointments and/or screenings.
- Communicate and collaborate with the individual's healthcare team, including, but not limited to, the facility Social Worker and Waiver Case Manager.
- Adheres to the Care Star Rule in performance of job responsibilities.
- Understands and complies with Care Star Policies and Procedures.
- Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
- Follows the Acceptable Use Policy while using any information systems owned or controlled by Care Star, Inc.
- At least 18 years of age or older.
- High School Diploma or higher education level.
- Minimum of 12 months of experience delivering services in a healthcare, behavioral health or social services setting.
- Experience advocating on behalf of individuals with chronic illness, mental health and/or substance use disorders, and physical disabilities.
