Nurse Practitioner - Geriatric Primary Care

This practice sees patients in assisted living facilities, independent living facilities, and private homes. They are hiring in the Youngstown area and will work with providers during the interview process to ensure a mutually‑agreeable geographic coverage area.

COMPENSATION
  • $125,000 to $170,000 Annual Income
  • $105,000 to $125,000 Base Salary
  • $20,000 to $45,000 Annual Bonus Income, paid quarterly

This organization is dedicated to delivering convenient, high‑quality primary care to geriatric patients in the comfort of wherever they call home. They currently boast a team of more than 25 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include, among others, primary care, psychiatric care, post‑acute care, and transitional care.

POSITION

DESCRIPTION

The nurse practitioner will serve as the primary care provider for a dedicated panel of geriatric patients located within a defined geographic territory within Youngstown. Patients are located in private homes (house calls), as well as long‑term care facilities (ALFs, ILFs).

Providers see a variable number of patients per day, dependent upon type of visit, travel time, and patient needs.

Dedicated schedulers put together daily schedules and routes for the providers to ensure efficiency and appropriate time for every visit.

Specific duties of the role include, but are not limited to:

  • Conduct health assessments to identify patient needs
  • Develop personalized care plans
  • Regularly evaluate patient conditions and responses to treatment
  • Adjust treatment plans as necessary
  • Perform minor procedures and wound care as required (and as settings permit)
  • Educate patients and families on health management, disease prevention, etc.
  • Provide guidance on lifestyle modifications and self‑care practices
  • Work with physicians, nurses, and other professionals to coordinate care

The practice uses a mobile‐based EHR with AI functionality to reduce administrative time.

On‑call is phone triage only with very low volume. Providers rotate the on‑call responsibility, with a shift every 12 weeks or less.

REQUIREMENTS
  • Ohio NP license
  • DEA
  • New grads with backgrounds in home health, LTC, ER, etc. are encouraged to apply
