Registered Nurse; RN - Hospice - Youngstown, Ohio

Position Summary

PRN Hospice Registered Nurse at Mercy Health Home Care and Hospice by Compassus provides clinical, psycho‑social, and case‑management care to hospice patients in the community and hospice house. The RN supports the interdisciplinary team and ensures patient comfort and family support.

• Complete routine and emergency assessments per policy.
• Provide clinical and psycho‑social services per plan of care.
• Record observations, treatments, and other pertinent information.
• Communicate with IDT, Medical Director, and attending physician regarding symptom management.
• Provide physical and emotional interventive care during crises.
• Prevent unwarranted hospitalizations by proactively addressing patient and family needs.
• Collaborate with IDT to coordinate hospice care and maintain continuity.
• Update the plan of care for the IDT.
• Document changes and submit required reports by 10 a.m. the next day.
• Perform duties as assigned.

• Associate or Bachelor’s degree in Nursing.
• Minimum 1 year of nursing practice or equivalent experience.
• Hospice, oncology, home health, or long‑term care experience preferred.

• Active RN license in state(s) of employment.
• Current CPR certification.

Regular standing, walking, and manual dexterity; lift up to 50 pounds. Visual acuity required. Exposure to bodily fluids and infectious diseases expected.
