The Nurse Practitioner is responsible for the provision of preventive medical, palliative, and educational services to promote patient health and wellness and uses standard software and the comprehensive clinical‑financial computer system ensuring compliance in accordance with company standards and federal, state, and local standards, guidelines, and regulations.
Responsibilities- Conduct comprehensive medical and social histories and perform physical examinations of healthy, acutely ill, chronically ill, or terminal patients.
- Treat common acute illnesses, chronic and stable health conditions through prescribing medications and other treatments.
- Order and interpret lab tests and x‑rays.
- Provide information, instruction, and counseling on health maintenance, health promotion, social problems, illness prevention, illness management, palliative care, and medication use.
- Refer patients to other health professionals as needed.
- Communicate with staff, patients and families to promote optimal patient outcomes.
- Complete documentation accurately and timely while maintaining state, federal and agency regulation.
- Participate in quality improvement projects.
- Commit to excellent customer service for internal and external customers.
- Perform any other duties as assigned.
- Current unencumbered license for the state of Ohio to practice as a Certified Nurse Practitioner (CNP).
- Current Certificate of Prescriptive Authority or Externship Certificate to prescribe.
- Two years experience in the home health and/or hospice field.
- Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet.
- Proficiency with electronic medical records (EMR) preferred.
- Must be able to read, write, speak, and understand the English language.
- Must possess a valid driver's license and acceptable driving record.
- Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.
- Sitting – up to 8 hours/day.
- Standing – up to 8 hours/day.
- Walking – up to 8 hours/day.
- Driving – up to 4 hours/day.
- Lifting, transferring, pushing or pulling patients or equipment/supplies – up to 50 pounds.
- Work weekends, evenings, and holidays – required as scheduled.
- Subject to falls, burns from equipment, odors, and cuts.
- Work day subject to patients with various disease processes.
- Occasional exposure to infectious waste, disease, conditions, etc., including exposure to AIDS and Hepatitis B viruses.
- Risk Category for Exposure to Bloodborne Diseases – I.
