About the RoleThe RN Case Manager plays a crucial role in coordinating patient care and ensuring that individuals receive the appropriate services to meet their health needs. This position involves assessing patient requirements, developing care plans, and collaborating with healthcare providers to facilitate effective treatment. The RN Case Manager will also monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
By acting as a liaison between patients, families, and healthcare teams, the RN Case Manager ensures that all parties are informed and engaged in the care process. Ultimately, the goal of this role is to enhance the quality of care and improve patient satisfaction through effective case management.
- Current Registered Nurse (RN) license in the state of practice.
- Bachelor's degree in Nursing (BSN) or equivalent experience.
- Minimum of 2 years of clinical nursing experience, preferably in case management or a related field.
- Certification in Case Management (CCM) or similar credential.
- Experience with electronic health records (EHR) and case management software.
- Knowledge of community resources and healthcare systems.
- Conduct comprehensive assessments of patients' health needs and develop individualized care plans.
- Coordinate with healthcare providers, specialists, and community resources to implement care plans effectively.
- Monitor patient progress and adjust care plans based on ongoing assessments and feedback.
- Educate patients and their families about health conditions, treatment options, and available resources.
- Document all interactions and updates in patient records to ensure continuity of care.
The required skills for this position include strong assessment and critical thinking abilities, which are essential for evaluating patient needs and developing effective care plans. Excellent communication skills are vital for educating patients and collaborating with healthcare teams, ensuring that everyone is aligned in their approach to care. Organizational skills are necessary to manage multiple cases and maintain accurate documentation. Preferred skills, such as familiarity with EHR systems, enhance efficiency in tracking patient progress and coordinating care.
Overall, both required and preferred skills contribute to the RN Case Manager's ability to provide high‑quality, patient‑centered care.
Monday through Friday with an on-call weekend rotation.
