Summary
The Advanced Practice Nurse (APN) Certified Nurse Practitioner provides a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The Nurse Practitioner provides advanced clinical practice, consultation, and management within the Hematology & Oncology section of the Medicine Service.
Duties- *** This position will have occasional coverage at the Kent - OH Clinic
*** VA Healthcare System Serving Ohio, Indiana and Michigan (VISN 10) advocates for a Whole Health System of care in each of the Medical Centers. - This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest.
- As an employee operating in a Whole Health System of care you will operate in a model with three core elements – seeking to create a personalized health plan for each Veteran.
- This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community.

- Major Duties:
Independently manage a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence‑based practice. - Develop and implement an individualized plan to achieve optimal population health and disease management.
- Demonstrate leadership and work collaboratively with the interdisciplinary team.
- Order and interpret labs, tests, and diagnostic studies.
- Prescribe pharmacologic and non-pharmacologic interventions.
- Initiate referrals and consults to specialty services.
- Provide education to patients and their families.
- Assess and adjust the plan of care as needed to achieve optimal patient outcomes.
