Communi Care Health Services seeks an RN MDS Nurse to join our team. The MDS Nurse coordinates MDS assessments, oversees the RAI process, and ensures appropriate clinical services and reimbursement for residents.
The role reports to the Executive Director and supports quality outcomes across the facility. Graduate of an accredited school of nursing with an active RN license in the state of employment is required; three years of long-term care experience and MDS/RAI familiarity are preferred.
