Briarfield Health Care Centers is seeking a
MDS NurseThe MDS Nurse will gather information on the building's current residents for future assessment, including physical and mental states. They assess charts and communicate with health care teams to create applicable health care plans for their current and incoming residents. The MDS Nurse will coordinate the delivery of the building's services to our residents. They will also observe and document the pricing and effectiveness of these services.
Job Duties:- 1. Based on the MDS 3.0, makes a comprehensive assessment of resident's needs which includes; prior medical function status; sensory and physical impairments; nutritional status; special treatments and procedures; discharge potential; dental condition; activities potential; rehab potential; cognitive status and drug therapy, assessment must be no later than 14 days after date of admission. All resident assessing will be under the direction of the Director of Nursing or the RN coordinator.
- 2. May be required, at the time each resident is admitted, to obtain physician orders for immediate care; ensure physician orders are received and signed as required, and enter in computer.
- 3. Updates physician orders in computer and provides Physician Orders (PO), Medication Administration Record (MAR), Treatment Administration Record (TAR) for monthly changeovers. Print out once complete.
- 4. Reviews MDS 3.0 assessments when there is a change in resident's condition; reviews MDS 3.0 assessment at least quarterly and yearly/annually.
- 5. Ensures that the resident's abilities in activities of daily living (ADL) do not diminish unless unaviodable; ADL's include ability to bathe, dress, groom, transfer, ambulate, toilet, eat and speak. Works in coordination with interdisciplinary team.
- 6. Discharge summary must be completed and a discharge plan of care developed when it is anticipated that resident is to be discharged. May be delegated to a RN, Head Nurse or Social Services. The discharge plan of care must be signed by an RN and all other disciplinary team members.
- 7. Maintain a positive attitude at all times
- 8. Other duties as assigned by supervisor.
- 9. Participates in the on-call rotation
- Current unencumbered RN or LPN license in the State of Ohio
- Current CPR Certifications.
- Must have at least 2 years' experience in long term care.
- Working knowledge of MDS, care planning and PPS rules preferred.
- Must have excellent inter-personal skills.
