Travel Social Work - Social Worker

Job Summary

Horizon Health is seeking a licensed Social Worker for a 13‑week travel assignment in Youngstown, Ohio. The position is full‑time, 40 hours per week, with day shifts and an enhanced hourly rate plus tax‑exempt stipend (where applicable).

• Provide individual, family, multi‑family, and group therapy as needed.
• Serve as liaison between treatment team, physicians, referral sources, community agencies, outside therapists, and third‑party reviewers.
• Complete assessments, contribute to master treatment plans, and manage discharge planning and summaries.
• Provide high‑quality patient care, assess functional needs and outcomes, consult with the treatment team.
• Regularly assess patient needs and assist in updating the Master Treatment Plan.
• Work within a team environment to ensure appropriate treatment modalities.
• Maintain all required documentation in a timely manner per policies and regulations.

• Master’s degree in Social Work or Counseling from an accredited school.
• Licensed in Ohio as LSW, LISW, LPC, or LPCC.
• Prior experience in a mental health care facility preferred.
• Knowledge of developmental and life‑stage psychiatric problems for ages 3‑18.
