What You'll Do In This Role: Start your nursing career at Huron Valley PACE! As an Extended Care Nurse at Huron Valley PACE, you’ll provide essential after-hours and weekend care, ensuring participants receive timely, compassionate support when they need it most. Working a unique 7-on-7-off schedule, you’ll take nursing and intake/referral calls, assess patient needs, and coordinate both scheduled and as-needed visits - helping manage complex conditions and prevent unnecessary hospitalizations.
In close collaboration with the interdisciplinary team, you’ll use your clinical expertise to assess, plan, and deliver care that enhances independence, promotes autonomy, and supports the overall well-being of the older adults we serve.
